Histopathological Study on the Gastrectomized Specimens Observed in the Sung Shim Hospital



Abstract
The stomach, as an upper part of the digestive tract, performs several important physiologic roles during the digestive process. And the stomach is frequently involved in psychosomatic disorders, because it has close correlationship with neuropsychiatric function. However, the most prevalent diseases observed in the gastrectomized specimens are benign and malignant ulcers during the pathologic examination. It has been reported that 10% of general population in U.S.A. are suffered from peptic ulcers. Although there are numerous studies on the cases of the gastric diseases, the frequency of the gastric ulcer and/or malignancy in general population are not yet discovered in Korea. The malignancy of the stomach is one of the most prevalent malignancies in Europe, in U.S.A. and even in Korea.
In order to clarify the frequency and relationship of the diseases of the stomach in the histopathologic viewpoint, the authors made a statistical studies on the histopathologic findings observed in the sub totally resected stomach from 16 June 1968 to 30 November 1976 (8 years and 5 months) in the Sung Sim Hospital, Chung-Ang University, Medical School.
The follows are ensued:
1. During the experimental period the total number of sub totally gastrectomized. cases- are 325. It is 0.82% (Peptic ulcer 0.466, Malignancy 0.35%) of total admitted. cases, 5.65% (Peptic ulcer 3.15%, Malignancy 2.40%) of admitted cases to the Dept. of Surgery and 4.65% (Peptic ulcer 2.60%, Malignancy 1. 98%) of all pathologically examined, cases.
2. The most prevalent diseases found in the sub totally resected stomach in this study are malignancy (138-cases, 42.5%). bud gastrc peptic ulcer (131 cases, 40.3%).
3. The gastric diseases in this study are more prevalent in- men than women. The duodenal ulcer patients in this study are all mostly male with sex ratio of 9 : 1.
4. The ulcerative lesions involving stomach have same predilection sites on :the lesser curvature of the antral portion, and the differentiation of benign ulcer from malignancy is almostly impossible on the ground of the predilection site of¢¥ the lesions.
5. There is a high suspicious of benign ulcer, if the diameter of the ulcer crater is less than 2 cm, and there is high suggestion for malignancy, if the diameter is larger than 4 cm on the gross examination.
6. The most cases of the gastric malignancy are ulcerative forms (117 cases, 85.4%) on the gross examination and simple adenocarcinoma (104 cases, 75.9%) on the histological examin-ation.
